• The information you provide on the second part of this registration form will be part of the PowerPoint presentation and the emcee’s script at the style show. 
• Please complete the questions before paying. You will not be able to go back and edit your answers so double check your answers! 
• The first 50 students who submit a completed registration form, payment, and photo will be in the show. 
• Please email a photo of yourself in a separate file to Mrs. Fischer at bfischer@incarnateword.org. Please send an appropriate photo that is already cropped to fit into a 2.75 x 2.75 frame on the PowerPoint slide.

Each student agrees to the following upon completion of this form:​

• To attend the model information session (time and date TBD) in February before the event. 
• To select an outfit from your assigned retailer at the assigned date/time the week before the show.  (Please consider modesty when selecting your outfit.)  
• No accessories or shoes may be borrowed from retailers. 
• To take extreme care of the outfit borrowed from the retailer.  (Any damages to the outfit will result in payment from the student/parent to the retailer.) 
• To arrive on time to the Junior League of Houston the morning of the show (exact time of requested arrival TBD).
